Instructions for Subscribing

  1. Fill out the form below and print out 2 copies (clicking the print icon twice on your browser's toolbar should do it).
  2. Once you have printed out this page with the form, please REMEMBER to press the "Submit" button and send the information to us.  [NOTE: You must fill out all but the optional fields (marked by *) to allow us to add you to our subscriber database.]
  3. Keep one of your printed copies for your records, and mail one printed copy (with your check) to us at the address below. Once we receive your printed form, we'll marry it up with the electronic submission, and e-mail a confirmatory e-mail with the beginning and end dates of your subscription.

Remember, only subscribers have access to the fund recommendations provided in the TSP Advisory's subscribers-only area.
